If you have been diagnosed with a mental health condition and are taking or considering mental health medication, your insurance may cover the cost of the GeneSight test.
*Reduced self-pay cost
If you decide to use your insurance and we determine that your cost will exceed $330, we will contact you before processing the test and before billing you to make sure you would like to proceed.
All patients are eligible for the reduced self-pay option of $330. Self-pay may be the most affordable option for patients whose out-of-pocket cost is estimated to be higher than $330, for those whose insurance does not cover the GeneSight test, or for those without insurance.
For Medicare, if the test is considered non-covered, your clinician will need to submit an Advanced Beneficiary Notice signed by you before we can deliver the GeneSight test. By signing the ABN, you agree to pay the full cost of the test and will be billed accordingly. Learn more about ABN’s here.

If your cost is $100 or more, we offer an interest-free payment plan that allows you to spread your cost out for up to 12 months, regardless of insurance coverage or household income.
Based on people who meet the U.S. Department of Health and Human Services federal poverty guidelines. Due to regulatory limitations, people who carry federally funded health insurance such as Medicare, Medicaid, TRICARE, and Medicare Advantage are not eligible for financial assistance. People with some types of Medicaid plans, including those with limited state-funded plans such as emergency-only coverage, or Medicaid in states that do not have coverage for Myriad testing, are eligible for the Myriad Financial Assistance Program; contact Myriad for details about your specific plan type.

When we receive your sample, we review your insurance benefits to estimate your out-of-pocket cost. If it could be more than $330, we will contact you before processing the test to discuss your options, including canceling the test at no cost to you.
If insured, we bill your insurance after processing your test. Your health insurance company will process our claim, then send you an EOB within 30-45 days. This is not a bill that you pay. The EOB or Explanation of Benefits is a detailed explanation of the amount your insurance has covered for the GeneSight test(s). Most EOBs require no action.

Once your claim is complete, you will receive a statement of what you owe, which will be sent via U.S. Mail from Myriad Genetics. You can either pay in our online portal or process with our billing team at 888-496-2391.
If insured, insurance processing takes time and it will likely be several months before you receive this bill from us. We will keep you in the loop along the way. Once you know what you’ll owe, you may also qualify for financial assistance to what your insurance does not cover and we offer an interest-free payment plan.
